cohealth are seeking a Mental Health & Wellbeing Worker to join a multidisciplinary team serving the Wyndham catchment in Victoria. The role delivers warm, trauma-informed, recovery-oriented support to adults 26+ with mental health challenges, alongside clinicians, lived experience staff and partner organisations.

You will contribute to brief interventions, wellbeing coaching, psychosocial support, group programs and social prescribing, with strong emphasis on safety planning and coordinated

Qualifications

• Able to form respectful, professional relationships with clear boundaries.
• Strong psychosocial assessment, engagement and support-planning skills.
• Experienced working with people experiencing distress, trauma, complexity or uncertainty.
• Strong belief and personal alignment in cohealth Values and Mission.

Responsabilités

• Provide brief intervention, wellbeing coaching and psychosocial support for people aged 26+ experiencing mental health challenges, psychological distress or psychosocial stressors.
• Support group programs, social prescribing activities, community engagement and partnerships with local services.
• Support navigation of mental health, AOD, family violence, housing, community health and social services.
• Complete risk assessments, safety plans and incident reports in line with cohealth policy.
