Job summary

HealthWISE in rural NSW and beyond is seeking a Program Officer for a 12-month contract to provide responsive intake, triage and referral management for mental health programs. You will be the first point of contact for GPs, referrers and clients, reviewing referrals, assessing eligibility and prioritising risk to connect people with the right support.

Qualifications

• Diploma or Certificate IV in Mental Health or equivalent.
• Experience in intake and triage within a health or mental health setting.
• Understanding of stepped care and client-centred practice.
• Ability to maintain accurate client information and documentation.
• Professional communication with clients, referrers, clinicians and partners.
• Ability to work independently within a multidisciplinary team.
• Cultural awareness and understanding of stigma and discrimination.

Responsibilities

• Review and process referrals, assess eligibility and risk.
• Provide responsive intake, triage and referral management support.
• Conduct telephone-based assessments when required and complete risk assessments.
• Maintain accurate client records and ensure data quality.
• Escalate complex or high-risk matters and coordinate with stakeholders.
• Support safe and timely access to mental health services.
