Job summary

HealthWISE is seeking a Program Officer for a 12-month contract to provide intake, triage and referral management support for mental health programs. You will be the first contact for GPs, referrers and clients, reviewing referrals, assessing eligibility and risk, and coordinating timely access to services across rural NSW and QLD.

The role requires strong communication, organisational and documentation skills, plus commitment to privacy and best-practice care.

Qualifications

• Diploma or Certificate IV in Mental Health or equivalent.
• Experience with intake and triage in health or community settings.
• Understanding of stepped care and referral pathways.
• Ability to maintain accurate client information and documentation.

Responsibilities

• Review and process referrals, assess eligibility and risk.
• Provide responsive intake, triage and referral management support.
• Conduct telephone-based assessments and safety planning where required.
• Escalate complex or high-risk cases and maintain client records.
