Job summary

Ballarat Community Health is seeking an Intake and Triage Clinician to be the first point of contact for individuals seeking support. You will listen, assess needs, and connect people with appropriate services through our no wrong door approach.

The role supports assessments and brief interventions, coordinates referrals, and collaborates with a multidisciplinary team to achieve positive outcomes for individuals, families and carers.

Qualifications

• Relevant qualifications and registration (where applicable) in Social Work, Occupational Therapy, Psychology, Mental Health Nursing or a related discipline
• An interest in mental health intake, assessment, care coordination and service navigation
• The ability to engage respectfully and effectively with people from diverse backgrounds and experiences
• Sound communication, relationship-building and problem-solving skills
• A willingness to learn, seek guidance and contribute to a collaborative team environment
• Experience in mental health, community services, intake, triage or care coordination will be highly regarded
• Knowledge of local mental health and community support services is advantageous

Responsibilities

• As an Intake and Triage Clinician you’ll be the first point of contact for people seeking support.
• You’ll take the time to listen, understand each person’s unique circumstances, assess their needs, and help connect them with the most appropriate services and supports.
• You’ll undertake assessments, provide brief interventions, support service navigation, coordinate referrals, participate in multidisciplinary discussions and contribute to positive outcomes for individuals, families and carers
