Job summary

Quantum Support Services is seeking an experienced case manager to provide intensive support to individuals and families exiting mental health facilities and facing homelessness. You will build strong relationships, advocate for clients, and coordinate housing, health and community services to improve wellbeing.

Qualifications

Diploma of Community Services or equivalent tertiary qualification is required.
Experience in case management, support coordination, community services, mental health, housing, or related human services.
Commitment to person-centred and recovery-oriented practice.
Valid Victorian Driver's Licence, Working with Children Check, and Police Check.

Responsibilities

Provide intensive case management and transitional support to individuals and families exiting mental health facilities and experiencing homelessness.
Build trusted relationships through assertive outreach to empower clients to achieve independent living.
Connect clients with housing, health, community and support services to improve wellbeing.
Collaborate with community agencies and stakeholders to deliver integrated, client-centred support across Gippsland.
