Job summary

Quantum Support Services is seeking an experienced case manager to provide intensive support to individuals and families exiting mental health facilities and facing homelessness. You will build strong relationships, advocate for clients, and coordinate housing, health and community services to improve wellbeing.

Qualifications

• Diploma of Community Services or equivalent tertiary qualification is required.
• Experience in case management, support coordination, community services, mental health, housing, or related human services.
• Commitment to person-centred and recovery-oriented practice.
• Valid Victorian Driver's Licence, Working with Children Check, and Police Check.

Strong relationship-building and communication skills, with the ability to engage clients, advocate effectively, and work collaboratively with a range of stakeholders.

Experience in case management, support coordination, community services, mental health, housing, or a related human services field.

Resilience, initiative and sound judgement, with the ability to manage a diverse caseload, work independently, and adapt to changing client needs.

Valid Victorian Driver's Licence |Working with Children Check |Police Check

Minimum requirement of Diploma of Community Services or other relevant or appropriate tertiary qualifications and/or experience relevant to this position.
