Liberty Community Connect is seeking a dedicated home and community support worker on the Gold Coast to assist elderly and disabled adults and their carers. You will deliver domestic assistance, social connection, personal care and respite while ensuring flexible, goal‑oriented care.

Applicants should hold a Certificate III in aged/disability/community services, have a valid NDIS workers clearance, CPR/First Aid, a current driver’s licence and a reliable insured vehicle.

Qualificações

• Certificate III in aged/disability/community services/individual support (must-have).
• Ability to interact, motivate and support people to meet their goals.
• Proficiency with mobile devices, phones and computers including accurate data entry.
• NDIS Worker Clearance required.
• CPR and First Aid qualifications.
• Driver's licence in resident state; reliable insured vehicle.
• Smartphone capable of rostering apps and related tasks.
• Vaccinations as directed by public health authorities.

The role is responsible for assisting people who are elderly, adults with a disability and/or their carers by delivering innovative high quality home and community support services (i.e. domestic assistance, social support/connection, personal care and/or respite). These duties will complement Liberty's approach that services and support will offer choice and flexibility ensuring that individual needs and goals are met with a vision of health and wellness.

• Minimum Certificate III in aged/disability/community services/individual support
• Expertise and competence sufficient to undertake the range of duties required
• Demonstrated ability to interact, motivate, inspire and support people to meet their goals
• Demonstrated good standard of proficiency in technology skills (e.g. mobile device, phone, computer) with accuracy in data entry
• NDIS Workers Clearance
• CPR and First Aid qualifications
• Driver's licence in the state of residency
• Reliable and comprehensively insured vehicle
• Reliable and operational smart phone suitable for the operation of rostering and other applications
• Skills, knowledge and physical capacity to carry out role
• Vaccinations as directed/mandated by Public Health Authorities/Government
