Job summary

A Good Start to Life is seeking a passionate Case Manager to lead a dedicated team in Mackay, working with children and young people with complex disability-related health needs. You will advocate for clients, develop care plans and ensure high-quality, person-centred care.

This full-time role focuses on leadership, collaboration with stakeholders, mandatory reporting and continuous improvement, shaping outcomes and the trajectory of young lives.

Qualifications

• Experience in children, youth and family services at a management or supervisory level.
• Certificate IV, Diploma or Degree in Child, Youth & Family Intervention, Disability or a related discipline.
• Strong understanding of Child Protection and Disability support; able to work with government and non-government agencies.

Responsibilities

• Leading and supporting a team of youth workers to deliver consistent, high-quality and person-centred care
• Developing, implementing and regularly reviewing individual care plans reflecting each young person’s needs and goals
• Collaborating with stakeholders, government agencies and community partners for coordinated, holistic support
• Ensuring mandatory reporting and administrative tasks are completed accurately and on time
• Participating in supervision, team meetings and stakeholder discussions to drive communication and continuous improvement
• Providing direct care support to young people when needed to maintain continuity of care
