Job summary

Based in Rockhampton or Gladstone, the Case Manager at DCC Industry Group’s Youth and Communities division delivers comprehensive case management for children and young people in non-family based care. You will supervise House Managers, champion trauma-informed practice, and ensure compliance with HSQS and Child Safety licensing standards.

You will develop tailored plans, monitor progress, advocate for rights, and build strong partnerships with government, NGOs, and families to secure resources

Qualifications

• Hold a tertiary qualification in social work, human services, psychology or relevant field.
• Minimum of 2 years’ experience in Child Protection in Queensland, including out-of-home-care.
• Ability to effectively communicate at all levels, influence, engage, and build.
• Demonstrated support and direct staff to achieve desired outcomes for young people.
• Developed time management and organisational skills.
• Excellent communication both written and oral.

Responsibilities

• Develop, implement, and review tailored case management plans with children, young people, and care teams.
• Monitor progress and adjust plans to ensure positive outcomes.
• Support transition planning (e.g., adulthood, housing).
• Ensure all care is trauma-informed, therapeutic, and safe.
• Address risks promptly and uphold compliance with policies and legislation.
• Advocate for children’s rights and ensure access to needed resources and services.
• Provide supervision, mentorship, and guidance to House Managers.
• Foster a collaborative, inclusive, and psychologically safe team culture.
• Promote professional development, self-care, and staff well-being.
• Implement performance management and encourage alignment with organisational values and goals.
• Build strong relationships with external partners (government, NGOs, families).
• Communicate regularly to align goals and coordinate resources.
• Ensure the voices of children, young people, and staff are reflected in service planning and delivery.
• Maintain accurate case records and documentation.
• Use relevant systems to track and report on outcomes.
• Ensure administrative tasks are timely and meet policy and legislative standards.
