Job summary

Kanda is recruiting a passionate Case Manager for the Good Start to Life team in Mackay. You will lead a dedicated group of youth workers, develop individual care plans and coordinate with government agencies to support children and young people with complex needs.

This full-time, hybrid role centers on leadership, advocacy and hands-on care, shaping outcomes and trajectories for the young people you serve.

Responsibilities

• Lead a team of youth workers to deliver high-quality, person-centred care.
• Develop, implement and review individual care plans for each young person.
• Collaborate with government agencies and community partners for coordinated support.
• Ensure accurate reporting and administrative tasks are completed on time.
• Participate in supervision, meetings and stakeholder discussions to drive continuous improvement.
• Provide direct care support when needed to maintain continuity of care.

We’re looking for a passionate, hands‑on Case Manager to join the Good Start to Life (GSL) team full‑time in Mackay. You’ll walk alongside children and young people with complex disability‑related health needs — supporting them, advocating for them and leading a dedicated team of youth workers who show up and do the same.

This is the kind of role where your leadership shapes culture, your care shapes outcomes and the work you do genuinely changes the trajectory of a young person’s life.

What you’ll be doing

• Leading and supporting a team of youth workers to deliver consistent, high‑quality and person‑centred care
• Developing, implementing and regularly reviewing individual care plans that truly reflect the needs and goals of each young person
• Collaborating with stakeholders, government agencies and community partners to ensure coordinated, holistic support
• Ensuring mandatory reporting and administrative tasks are completed accurately and on time, every time
• Actively participating in supervision, team meetings and stakeholder discussions to drive communication and continuous improvement
• Stepping in to provide direct care support to young people when needed to maintain continuity and consistency of care

You're the right person if…

• You have demonstrated experience in children, youth and family services or a similar sector at a management or supervisory level — and you lead with heart as much as skill
• You hold a Certificate IV, Diploma or Degree in Child, Youth & Family Intervention, Disability or a related discipline
• You have a strong understanding of Child Protection and Disability support and are confident working alongside government and non‑government agencies
• You’re a natural leader with excellent decision‑making and interpersonal skills who brings out the best in the people around you
• You hold a valid Driver’s Licence, Working with Children Check, NDIS Worker Screening Clearance and LCS2 Approval (or willing to obtain)
• You are willing to travel to Townsville as required to support with building services in the region
