Job summary

Act for Kids is seeking an Intake and Family Support Practitioner for the Rockhampton Intensive Family Support team. You will be the first point of contact for referred families and provide direct case management to help children and families achieve positive outcomes.

The role involves assessment, engagement, and evidence-informed interventions to connect families with the support they need to thrive, with a level 4 SCHCADS Award and a salary range of $92,287 - $99,295 plus super.

Qualifications

• Two+ years’ experience providing case-management and therapeutic services to families and children.
• Eligible to work in Australia with Working with Children Check or exemption.
• Registration or membership in a professional association is desirable.

Responsibilities

• Support families from first contact to positive outcomes through referrals and needs assessment.
• Deliver evidence-informed case management interventions.
• Promote child safety and wellbeing through risk assessment and safety planning.
• Build trusted relationships with families and community partners to coordinate support.
