Job summary

Health and Wellbeing Worker role in Caboolture with IUIH Institute for Urban Indigenous Health. Full time position focused on culturally responsive holistic primary health care and navigation for clients and families.

The role supports autonomy, education, and goal-driven planning within a multidisciplinary team to improve Aboriginal and Torres Strait Islander health outcomes in our community.

Qualifications

• Aboriginal or Torres Strait Islander person is essential.
• Current Criminal History Check.
• Current C Class Drivers Licence (QLD).
• Current Working with Children’s Check (QLD Blue Card).
• Current CPR Certificate.

Responsibilities

• Welcoming, empowering, and connecting clients and families to the service.
• Providing health and social determinant information and education.
• Work with clients in goal setting and prioritisation of health and social needs.
• Implementing plans in collaboration with the core multidisciplinary team.
