Job summary

Institute for Urban Indigenous Health Ltd is seeking a Health Wellbeing Worker in Caboolture, QLD. The role acts as a familiar face and navigator for Aboriginal and Torres Strait Islander clients, guiding access to culturally responsive holistic primary health care within a multidisciplinary team.

Responsibilities include welcoming clients, providing health information, supporting goal setting, and implementing care plans to improve health outcomes for the community IUIH serves.

Qualifications

• Aboriginal or Torres Strait Islander status is a genuine occupational requirement.
• Current Criminal History Check.
• Current C Class Driver's Licence (QLD).
• Current Working with Children’s Check (QLD Blue Card).
• Current CPR Certificate.

Responsibilities

• Welcoming, empowering, and connecting clients and families to and back into the service.
• Providing health and social determinant information and education to clients and families to promote autonomy and choice.
• Work with clients and families in goal setting and prioritisation of health and social needs.
• Implementing plans, based on goal setting, in collaboration with the core multidisciplinary team.
