Job summary

Silverchain Group is seeking a Care Partner to support older Australians in their homes across the Blacktown region. The role blends community-based visits with autonomy to work from home, focusing on person-centred care and positive outcomes.

You will assess needs, coordinate services and advocate for clients while collaborating with clinical and scheduling teams to deliver coordinated care.

Qualifications

• Experience in community-based case management.
• Experience supporting clients receiving government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Demonstrated empathy, compassion and a client-centred approach.
• Effective problem-solving and critical thinking skills.
• Well-developed organisational and time management skills.
• Ability to work collaboratively across multidisciplinary teams.
• Advocacy skills to support positive client outcomes.
• Current driver's licence and a reliable vehicle with appropriate insurance.

Responsibilities

• Manage a caseload of older Australians receiving government-funded aged care services in their homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor and manage client budgets in line with government program requirements.
• Coordinate services to ensure clients receive the right support at the right time.
• Support new clients through onboarding and ongoing service delivery.
• Build strong relationships with clients, families and key stakeholders.
• Work closely with clinical, scheduling and service delivery teams to deliver coordinated care.
• Advocate for client needs and support positive health and wellbeing outcomes.
