Job summary

Anglicare North Queensland Limited is recruiting a Cairns-based Advance to Zero and Intra-Agency Referrals Coordinator. The role drives a place-based, data-driven initiative to end homelessness by coordinating resources, campaigns and stakeholder engagement.

You will enable lived experience involvement and advocate for system improvements. You will oversee referrals across Anglicare NQ services to ensure timely connections and integrated care, while improving data quality and client outcomes

Qualifications

• Tertiary qualifications in Community Services / Social Sciences or related discipline.
• Significant experience in case management, service system advocacy and/or program/project management.

Responsibilities

• Oversee incoming referrals across Anglicare NQ services to connect clients to appropriate services in a timely way.
• Coordinate referrals to optimise resource use, streamline casework and improve service integration.
• Contribute to data quality, performance monitoring and client outcomes.
