Job summary

Junction Support Services in Victoria is seeking a Homes First Case Manager to support individuals and families experiencing homelessness to secure and maintain safe housing using Housing First principles. You will assess needs, develop support plans, provide advocacy and connect clients with health and community services.

Based in Wodonga on a full-time maximum term contract until 30 June 2027, you will work with a multidisciplinary Homes First team, maintain records, participate in supervision

Qualifications

• Experience in trauma-informed, strength-based case management.
• Ability to assess needs and develop individualized support plans.
• Ability to advocate for clients and connect with community services.
• Understanding of Housing First principles and culturally safe practice.
• Experience working with vulnerable communities who experience homelessness.

Responsibilities

• Deliver person-centred, trauma-informed and strengths-based case management.
• Assess client and family needs and develop, implement and review support plans.
• Support clients to secure and sustain long-term housing through practical assistance and advocacy.
• Build partnerships with clients, families and community services to coordinate supports.
• Maintain accurate case records, participate in supervision and comply with policies.
