Job summary

Valmar Support Services is seeking to enhance the independence and wellbeing of older clients through personalized in-home and community-based care. The role involves delivering personal care, domestic assistance, social support, transport, and respite under CHSP and HCP programs, within a supportive and compliant team.

Applicants should have a Certificate III in Individual Support (Aged Care), current First Aid/CPR, and a valid Australian driver’s licence.

Qualifications

Demonstrated ability to follow support plans while maintaining flexibility to respond to changes in client condition or needs.
Ability to provide domestic support, personal care, respite and social support (individual and groups) to clients with complex needs.
Understanding and application of wellness, reablement, and restorative care principles.
Capacity to manage physical demands of the role, including manual handling, lifting, and transferring clients safely.

About the Role:

The purpose of this role is to provide high-quality, person-centred support services to older individuals under the Community Home Support Program (CHSP), Home Care Packages (HCP), and requested Brokered Services. The role focuses on enhancing client independence, wellbeing, and community participation through effective delivery of services such as personal care, domestic assistance, social support, transport, and respite while contributing to a safe, collaborative, and compliant work environment.

Key Duties/Responsibilities

Without limiting the general scope and application of this position description, the duties include:

Direct Client Support

Provide individually negotiated services under the CHSP, HCP, Brokerage frameworks for clients in areas such as:
- Social Support
- Community Transport
- Domestic Assistance
- Personal Care
- Respite
- Garden Maintenance
Drive Valmar vehicles and operate machines, where licenced and where required
At all times, and in all locations, work with the clients in a positive and age-appropriate manner in line with the Valmar Code of Conduct
Ensure the concept and principles of wellness, enablement, and restorative care are understood and moulded to suit the individual circumstances of clients and service settings
Assist with medication reminders and prompts (as per care plan and scope of practice)
Assist clients with mobility and use of mobility aids (e.g., walking frames, wheelchairs)
Provide companionship and emotional support to reduce isolation
Support clients in engaging with the community and meaningful activities
Assist with errands such as grocery shopping, pharmacy runs, and appointments
Promote independence through daily living skills and supportive conversation

Compliance, Monitoring & Reporting

Ensure services are delivered as per funding guidelines and regulatory frameworks
Ensure clients, carers, and other support persons have avenues for providing input into the service delivery arrangement and negotiations
Maintain client documentation
Complete Incident reports as required
Participate in the Valmar Performance Appraisal process

Safety, Risk & Vehicle Responsibilities

Promote and encourage all aspects of safety within the home and the community
Work within Valmar WHS guidelines
Conduct vehicle maintenance checks on fleet vehicles as required
Use safety procedures when transporting clients, mobility equipment, and goods
Identify and report risks or hazards in the client’s home (e.g., fall risks, unsafe conditions)
Monitor client wellbeing and report significant changes in physical, emotional, or cognitive status to supervisors
Ensure heating/cooling, hygiene, and general home safety standards are met
Assist with infection control practices (e.g., PPE use, hygiene practices)

Teamwork & Communication

Contribute to team-based activities, meetings, and decision-making
Communicate effectively with clients, families, carers, and external stakeholders
Liaise with health professionals and community services when required
Demonstrate professionalism, empathy, and respect at all times, adhering to the organisation’s Code of Conduct and policies

Other:

Complete any reasonable requested tasks to support and enhance client-centred care

Selection Criteria:

Demonstrated ability to follow support plans while maintaining flexibility to respond to changes in client condition or needs.
Ability to provide domestic support, personal care, respite and social support, (individual and groups) to clients with complex needs.
Understanding and application of wellness, reablement, and restorative care principles.
Capacity to manage the physical demands of the role, including manual handling, lifting, and transferring clients safely.
Sound knowledge of safe work practices and the ability to identify and use safety equipment correctly.
Ability to work independently with only general supervision by a Coordinator
Strong administrative skills for accurate record keeping, documentation, and incident reporting.
High level of interpersonal and oral communication skills to engage effectively with clients, carers, and team members.
Administrative skills and basic digital literacy for using mobile apps and devices for record keeping and reporting.
Familiarity with aged care quality standards and regulatory frameworks.
Commitment to working within Work Health and Safety (WHS) and aged care compliance frameworks

Qualifications, Licences and minimum experience requirements:

Holds, or is actively working towards, a minimum Certificate III in Individual Support (Aged Care) or a related field, with a willingness to undertake further relevant training.
Experience providing a range of support services such as domestic assistance, personal care, social support (individual and group), transport, and short-term respite for carers.
Current First Aid and CPR Certificate.
Current and valid Australian driver’s licence.
Demonstrated experience working with older people , including those with complex needs.
Willing to get Criminal record checks
