Job summary

Harbour At Home in the Mid North Coast is seeking a passionate Care Partner to support aged care participants under the Support At Home program. You will build trusted, face-to-face relationships with participants and families, guiding them through care journeys with empathy and practical planning.

You’ll work across planning, budgeting and service decisions, staying close to participants’ goals while navigating funding requirements and compliance.

Qualifications

• Experience understanding Home Care Packages or Support at Home (desirable)
• Ability to manage a caseload and take ownership of client outcomes
• Strong communication and relationship-building skills
• Ability to balance client advocacy, funding requirements and compliance
• Comfort using digital systems and care management tools

Responsibilities

• Translate participant goals into personalised, compliant care plans
• Keep care plans current through reviews and updates
• Plan and track participant budgets to support goals and value for money
• Guide participants in decisions about services and funding
• Identify risks and changes in wellbeing and act timely
• Coordinate services and referrals to ensure seamless care
• Maintain accurate, audit-ready documentation
• Manage incidents, feedback and complaints professionally
• Work with care workers, clinicians and internal teams
• Use digital tools to stay organised and meet timeframes
