Job summary

UPA Aged Care Solutions in NSW is hiring a Home Care Partner to help clients remain independent at home and stay connected to their community. You will co-design care plans with clients and support the care team to achieve positive outcomes.

The role requires compassionate communication, coordination, and knowledge of Aged Care standards. A relevant qualification, driver’s licence, and pre-employment screening are essential. Join a not-for-profit team focused on dignity and quality of life.

Qualifications

• Tertiary qualification or equivalent experience in Aged Care, Community Services or a health-related discipline.
• Excellent communication and interpersonal skills.
• Experience in coordination, case management and care planning.
• Knowledge of Aged Care legislation, Home Care Standards.
• Competency using Microsoft Office and electronic client records.
• Current Driver Licence.
• Willingness to undergo Criminal History Screening (conducted by UPA).
• First Aid Certificate (or willingness to obtain).

Responsibilities

• The Home Care Partner co-designs care plans with clients based on goals, preferences and assessed needs.
• Supports functional independence, wellness and lifestyle choices and liaises with stakeholders to achieve positive client outcomes.
