Job summary

Sugarman Health & Wellbeing is seeking a Care Coordinator to coordinate individualised care for service users in their own homes and in the community. You will manage resources, ensure care meets policy, and work with multidisciplinary teams to deliver wellbeing services.

DBS is required and Training Trainer with Level 3 in Adult Care are preferred qualifications. Career progression to Senior Care Coordinator is available.

Qualifications

• DBS check required.
• Qualification matched to this role: Train the Trainer and Level 3 in Adult Care
• Experience coordinating care in community/home care settings is desirable.

Responsibilities

• Ensure sufficient workers available to meet client/service user requirements.
• Remains up to date with changes in legislation and group policies.
• Monitor health and wellbeing of Service Users with multidisciplinary teams.
• Maintain worker records in line with procedures and standards.
• Communicate changes to families and responsible bodies as needed.
• Support safe working practices and manage relationships with Service Users and families.
