Job summary

HammondCare is seeking a Full-time Care Partner on a 6‑month contract ending April 2027 to join the Home Care Team based in Hammondville, NSW. You will deliver tailored care plans and oversee ongoing assessments to align care with client needs and funding.

You should have a Certificate IV in Aged Care, at least 2 years of relevant experience, and be capable of working across multiple sites with a valid driver’s license. Competitive remuneration and additional benefits are provided.

Qualifications

• Certificate IV in Aged Care or equivalent is required.
• Minimum 2 years of relevant industry experience.
• Knowledge of Home Care Packages Program, CHSP, Aged Care Quality Standards, Commonwealth Home Support Programme.
• Strong administrative and computer skills.
• Current driver’s license and willingness to travel between HammondCare sites.

Responsibilities

• Deliver and develop tailored Care Plans for each client.
• Oversee ongoing assessment of clients’ needs and preferences.
• Manage care of existing clients and monitor client-related KPIs.
• Ensure care plans are up to date and aligned with funding and approvals.
