Job summary

HammondCare in Hobart, TAS, is seeking a full-time Care Partner to deliver and develop tailored care plans for clients in home care. You will oversee ongoing client assessments, coordinate with stakeholders, and ensure plans remain aligned with funding and service expectations.

Requirements include a Certificate IV in Aged Care, 2+ years’ experience, knowledge of Home Care Packages and Aged Care Standards, and a valid driver’s licence with willingness to travel between HammondCare sites.

Qualifications

• Certificate IV in Aged Care and a minimum of 2 years relevant industry experience.
• Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
• Current driver’s licence and willingness to travel between HammondCare sites and clients.

Responsibilities

• Deliver and develop tailored care plans for each client.

• Provide professional day-to-day delivery of services in line with service expectations and budgets.
