Job summary

HammondCare – Home Care Team in Hammondville is seeking a Full-time Care Partner on a 6-month contract ending April 2027. You will deliver and develop tailored Care Plans for each client and oversee ongoing assessment of needs within Home Care.

The role requires a Certificate IV in Aged Care with at least 2 years of experience, knowledge of Home Care Packages, CHSP and Aged Care Standards. You will travel between sites and support staff to meet funding and service expectations.

Qualifications

• Certificate IV in Aged Care and a minimum of 2 years relevant industry experience.
• Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
• Experienced in operating effectively in a multi-site environment.
• A current driver’s licence and willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request).
• Strong administrative and computer skills.

Responsibilities

• Deliver and develop tailored Care Plans for each client on the program and oversee the ongoing assessment of clients’ needs and preferences within Home Care.
• Care management of existing clients and client-related KPIs. Ensure care plans are up to date and aligned with funding and service expectations.
• Provide professional, day-to-day delivery of services to clients in line with service expectations and support staff to understand clients’ needs and preferences.
