Job summary

HammondCare in Miranda, Australia, is seeking a Full-time Care Partner to join our Home Care Team. You will deliver and develop tailored Care Plans for each client and oversee ongoing assessments of clients’ needs and preferences within Home Care.

The role involves care management of existing clients, ensuring plans stay up to date and aligned with funding, delivering services to a high standard, and collaborating with stakeholders.

Qualifications

• Certificate IV in Aged Care and minimum 2 years relevant industry experience.
• Knowledge of Home Care Packages Program, CHSP services, and Aged Care Standards.
• Willingness to travel between HammondCare sites and clients.

Responsibilities

• Deliver and develop tailored Care Plans for each client.
• Oversee ongoing assessment of clients’ needs and preferences within Home Care.
• Care management of existing clients and monitoring client-related KPIs.
• Ensure care plans are up to date and aligned with funding and service expectations.
