Job summary

The Royal Melbourne Hospital invites applications for a Case Manager in the Transition Care Program – Home Based at the Parkville Campus. You will co-ordinate short-term, goal-focused care for older adults after hospital discharge, working with patients, families and health professionals to optimise independence and support safe home living.

You will conduct assessments, liaise with service providers for in-home services, and help clients access ongoing supports as they transition to long-term

Qualifications

Qualified Registered Nurse or Allied Health professional with eligible registration.
Sound clinical skills and adaptable approach to meet client needs.
Understanding of goal-directed care and community programs.
Strong verbal and written communication with clients and professionals.
Solution-focused approach and problem-solving skills.
Interest in RMH community services knowledge.

Responsibilities

Conduct assessments over the phone and in home to assess risk and goals.
Liaise with service providers to deliver short-term home services.
Assist clients in engaging with ongoing supports and adapt to long-term care.
