Job summary

The Royal Melbourne Hospital – Parkville VIC is seeking a Clinical Nurse Consultant Coordinator for the Older Person Care Hub (OPCH). You will lead referral triage and service coordination to deliver timely face-to-face assessments within 24 hours and work with geriatricians and allied health staff to develop individualised care plans.

The hub operates five days a week with expansion to seven days planned for 2027; the role includes weekend shifts approximately every third weekend and

Qualifications

• Registered Nurse with at least five years of post-registration acute experience.
• Strong clinical assessment, reasoning, problem-solving and prioritisation skills.
• Excellent written and verbal communication with accurate clinical documentation.
• Ability to work independently and collaboratively in a multidisciplinary setting.
• Experience building relationships with multidisciplinary teams and stakeholders.
• Understanding of chronic disease management and community care, preferably in acute geriatric or acute community care.

Responsibilities

• Triage referrals and oversee the day-to-day running of the Older Person Care Hub.
• Ensure timely face-to-face assessments within 24 hours of referral.
• Collaborate with geriatricians and allied health to develop and monitor care plans.
• Coordinate the four-week care pathway and follow-up for safe transitions.
• Maintain effective communication with families, GPs and other health services via telehealth and EMR.
