Job summary

The Royal Melbourne Hospital is seeking a part-time Clinical Nurse Consultant Coordinator for the Older Person Care Hub in Melbourne. This role focuses on leading triage, service coordination and timely assessments to help older adults remain safely at home and avoid unnecessary hospital visits.

As part of the team, you will work with geriatricians and allied health staff, oversee the four-week care pathway and support expanded seven-day operations from 2027, including occasional weekend shifts.

Qualifications

• Registered Nurse with post-registration acute experience.
• Ability to work autonomously in a community-focused setting.
• Strong clinical assessment and decision-making abilities.
• Proficient in coordinating care plans and communicating with multiple stakeholders.

Responsibilities

• Triage and receive referrals from VVED, ED and partners as the service expands.
• Manage day-to-day running of the Older Person Care Hub.
• Collaborate with geriatricians and allied health to implement care plans.
• Coordinate face-to-face assessments within 24 hours of referral; arrange follow-up via telehealth or phone.
• Oversee a structured four-week care pathway and safe home transitions.
• Maintain clear communication with families, GPs and health services, using EMR and written summaries.
