Job summary

The Royal Melbourne Hospital’s Royal Park Campus is seeking a Case Manager for the Transition Care Program - Home Based. You will coordinate short-term, goal-directed care for older adults following hospital discharge, working with clients, families and health professionals to support independence and safe home living.

This fixed-term role (until January 2027) offers a 64-hour fortnight schedule across four days, with a collaborative multidisciplinary team.

Qualifications

• Qualified Registered Nurse or Allied Health professional, eligible for relevant professional registration.
• Sound clinical skills and the ability to adapt your approach to meet client needs.
• Understanding of goal-directed care and community-based programs and services.
• Strong verbal and written communication skills to work effectively with clients, carers, healthcare professionals and service providers.
• A solution-focused approach with well-developed problem-solving skills.
• Interest in expanding your knowledge of community services within RMH and the broader community.

Responsibilities

• Conduct assessments over the phone and in the client’s home to assess risk, collaboratively establish goals and monitor progress.
• Liaise with service providers to implement short term services and therapy for clients in their own home.
• The TCP Case Manager will assist clients in engaging with ongoing supports and services, and help them adjust to long-term care arrangements.
