About us

Moreton ATSICHS operates a growing network of healthcare clinics for local Aboriginal and Torres Strait Islander people. Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. We bring together primary care, allied health, dental, mental health and wellbeing, maternity and early childhood, disability services, specialist services, and aged care as one team focused on clients’ individual needs and goals.

We are an integrated team working together to improve the health and wellbeing of our Community. Our network is growing across Caboolture, Morayfield, Strathpine, Deception Bay, and Margate – now there’s an opportunity for you to be part of a great team making a real difference in people’s lives.

About the role

As Health Wellbeing Worker, your role is the familiar face, connector point, and navigator for which Mob can ask for help and guidance in their autonomous access and use of culturally responsive holistic primary health care. • Welcoming, empowering, and connecting clients and families to (and back into) the service.
  • Providing health and social determinant information and education to clients and families which promotes autonomy and choice.
  • Work alongside clients and families in their goal setting and prioritisation of health and social needs.
  • Implementing plans, based on goal setting, in collaboration with the core multidisciplinary team.
