As Care Coordinator – MATSICHS Social Health, your role is to provide coordination of supports to Aboriginal and Torres Strait Islander clients experiencing complex social and mental health challenges.
You will undertake structured intake, screening, and triage support, provide brief psychosocial support within scope, and work collaboratively with clinical practitioners where higher‑level assessment or intervention is required.
