Gunnedah Shire Council in Gunnedah, NSW is seeking a Clinical Care Partner for GoCo in-home care services. The role reports to the In-Home Care Team Leader and focuses on comprehensive needs assessments and care plan development to support participants living at home.
The successful candidate will be an AHPRA-registered RN with aged or community care experience, capable of coordinating services across multidisciplinary teams and maintaining confidentiality.
Contract Type: Full Time, 35 hours per week
Salary Range: Grade 13 ( $1679.70 - $1854.07 per week + 12% Superannuation)
