Job summary

Gunnedah Shire Council in Gunnedah, NSW is seeking a Clinical Care Partner for GoCo in-home care services. The role reports to the In-Home Care Team Leader and focuses on comprehensive needs assessments and care plan development to support participants living at home.

The successful candidate will be an AHPRA-registered RN with aged or community care experience, capable of coordinating services across multidisciplinary teams and maintaining confidentiality.

Qualifications

  • Must be an AHPRA registered RN.
  • Experience in aged, disability or community care.
  • Proficient in Microsoft Office applications.

Responsibilities

  • Conduct participant assessments in line with the Aged Care Act.
  • Develop care and support plans with participants and supporters.
  • Coordinate service delivery and monitor budgets/fees income.
  • Collaborate with multidisciplinary teams to ensure integrated care.

Salary Range: Grade 13 ( $1679.70 - $1854.07 per week + 12% Superannuation)

Our Next Clinical Care Partner

Reporting to the GoCo In-Home Care Team Leader, The Clinical Care Partneris an integral part of our team and supports the commitment we make to our community in providing efficient, responsive, open and accessible services to them.

Key Responsibilities for this role include:
  • Participant Assessments: Conduct comprehensive assessments of our Participants' healthcare needs, considering the requirements outlined in the Aged Care Act.
  • Develop care and support plans directly with the participant and/or their supporters (where appropriate), that:
  • are based on Consumer Directed Care principles.
  • support the lifestyle choice of the participant to remain at home and in the community.
  • achieve productivity goals.
  • based on feedback from participants, their supporters, health professionals and service providers.
  • Ensure confidentiality, privacy, dignity, and endeavour to protect from discrimination or abuse. This also includes fulfilling requirements in relation to mandatory reporting (i.e., child protection and elder abuse).
  • Coordinate, monitor and report on service delivery, individual package budgets and fees income.
  • Work towards GoCo’s participants receiving culturally safe, equitable access to the service regardless of geographical location, membership to a particular social group or any other factor which may have the potential to result in marginalisation.
  • Report regularly to the GoCo In-Home Care Team Leader through formal and informal support and supervision to ensure work practices, service provision and legislative requirements meet best practice standards.
  • Collaborative Approaches: Collaborate with multidisciplinary teams, including medical professionals, allied health practitioners, and support facilitators, to ensure integrated care and support for our participants.
  • Provide quality administration support as required.

To qualify for this role, you will need to be an AHPRA registered RN.

Additional Key requirements for this position include:
  • Experience in aged, disability or community care
  • Demonstrated skills and knowledge in use of Microsoft Office applications.
  • Current valid NSW Class C Drivers Licence (ongoing employment is conditional on maintaining this Licence) and
  • Criminal Record Check.
  • Ability and continued commitment to work according to Councils' policies and Code of Conduct, with or without supervision.
  • Demonstrated ability to effectively communicate with vulnerable people and their carers and with people of Aboriginal and Torres Strait Islander descent.
  • Demonstrated ability to support people to achieve goals related to their quality of life and ability to live independently.

* While not required, desirable characteristics of a successful candidate could include:

  • Experience in Local Government.
  • Auditing experience
  • Diploma/Certificate in Case Management, or a related discipline.
  • Certificate IV/or Diploma in Aged Care and/or Dementia Care.
