Job summary

Coonamble Aboriginal Health Service – Dubbo NSW is seeking a dedicated Care Coordinator to ensure high‑quality, person‑centred care. You will coordinate service delivery, roster staff, and liaise with management, clinical teams and families to keep care responsive and coordinated.

The role requires Aboriginal or Torres Strait Islander identity, relevant AQTF certification, and experience in care coordination or health settings. Pre‑employment checks are mandatory.

Qualifications

• Identify as Aboriginal or Torres Strait Islander with ability to provide proof of Aboriginality.
• AQTF Certificate III in Aboriginal and/or Torres Strait Islander Primary Health Care, Human Services, Social Work or related field.
• Experience in care coordination or support coordination, preferably in Aboriginal or Torres Strait Islander community.
• Effective interpersonal skills, including written and verbal communication.
• Strong planning and organisation skills, and problem-solving skills.
• Background in office administration duties, particularly in a clinical setting.
• Demonstrates commitment to code of conduct, EEO, WH&S and cultural safety framework.
• Current checks: national criminal history, NSW Working with Children’s Check, unrestricted license.

Responsibilities

• Coordinate service delivery and maintain continuity of care.
• Roster and schedule staff to support participants.
• Work with senior management, clinical teams and support workers.
• Foster positive relationships with participants, families and stakeholders.
• Identify and resolve service delivery challenges and ensure effective communication.
