Job summary

Marathon Health is seeking a Care Coordinator or Aboriginal Health Worker to improve integrated care in the Wagga Wagga and Gundagai/Tumut Region (Wiradjuri Country). The role focuses on coordinating high-quality care for people with chronic diseases, collaborating with GPs and community teams to develop tailored plans.

Applicants should have Enrolled Nurse registration or be an Aboriginal Health Worker, with a commitment to ongoing development and culturally safe care.

Qualifications

• Registration as an Enrolled Nurse and/or as a qualified Aboriginal Health Worker.
• Proven experience in conducting comprehensive assessments, developing care coordination plans, delivering safe and responsive care, and evaluating outcomes.
• Strong understanding of caring for individuals with chronic conditions and navigating related challenges.
• Commitment to ongoing professional development and a person-centered approach.

Responsibilities

• Coordinate high-quality care for individuals with chronic diseases and complex health conditions.
• Collaborate with patients, families, carers and primary care teams to create tailored care plans.
• Work with GPs and other professionals to enhance planning and outcomes.
• Partner with local health teams to streamline services and ensure seamless care.
• Communicate clearly with patients, carers and community, seeking feedback to ensure usefulness.
• Improve access to allied health and specialists and culturally appropriate resources.
