Job summary

Marathon Health in NSW invites applications for a Care Coordinator or Aboriginal Health Worker to join our Integrated Care team in Wagga Wagga and Gundagai/Tumut Region. This permanent full-time role focuses on coordinated chronic disease care and culturally safe services.

You'll work with GPs, community nurses and Aboriginal medical organisations to deliver person-centred plans, with opportunities for professional development and wellbeing support.

Qualifications

• Registration as an Enrolled Nurse or Aboriginal Health Worker.
• Experience in care coordination and comprehensive assessments.
• Ability to engage respectfully with Aboriginal and CALD communities.

Responsibilities

• Coordinating high-quality care for individuals with chronic diseases and complex health conditions.
• Collaborating with patients, families, carers, and the primary care team to create tailored care plans.
• Working with GPs and other professionals to enhance care planning and outcomes.
• Partnering with local health teams to streamline services and ensure seamless care coordination.
• Communicating clearly with patients and communities, seeking feedback to improve care.
• Enhancing access to allied health, specialists, and culturally appropriate resources.
