Job summary

Coonamble Aboriginal Health Service – Dubbo NSW invites applications for a full-time Aboriginal Health Practitioner to support the clinical team in delivering comprehensive primary health care.

Successful candidates will identify as Aboriginal or Torres Strait Islander and must provide proof of Aboriginality. AQTF IV (or current study) and 715 health assessments knowledge are essential assets for this role.

Qualifications

• Must identify as Aboriginal or Torres Strait Islander with proof of Aboriginality (mandatory).
• AQTF Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice (or currently studying).
• Demonstrated ability to conduct 715 health assessments and knowledge of chronic disease management.
• Knowledge and experience using Best Practice appointment system, or willingness to learn.

Responsibilities

• Work closely with the clinical team to deliver primary health care.
• Provide clinical and primary health care for individuals, families and community groups.
• Coordinate and provide health care within the Dubbo community and ensure safety and quality standards.
