Job summary

Marathon Health is seeking Care Coordinators or Aboriginal Health Workers for permanent and fixed-term full-time roles in the Wagga Wagga and Gundagai/Tumut Region of NSW (Wiradjuri Country). You will work with GPs, nurses and Aboriginal medical teams to coordinate care for clients with chronic diseases and complex health issues.

Qualifications

• Registration as an Enrolled Nurse and/or as a qualified Aboriginal Health Worker.
• Experience in assessments, care coordination plans, safe and responsive care, and evaluating outcomes.
• Ability to engage with Aboriginal and Torres Strait Islander communities and CALD populations.

Responsibilities

• Coordinate high-quality care for individuals with chronic diseases and complex health conditions.
• Collaborate with patients, families, carers, and primary care teams to implement tailored care plans.
• Work closely with GPs and other medical professionals to enhance care planning and outcomes.
• Partner with local health teams to streamline services and ensure seamless care coordination.
• Communicate clearly with patients, carers, and the community and gather feedback to improve support.
• Enhance access to allied health, specialists, and culturally appropriate resources.
