Job summary

KLC Recruitment is seeking an experienced Nurse Practitioner to support GPs in Residential Aged Care Facilities from Batemans Bay to Dalmeny, NSW. The role offers flexible scheduling, including the option to work from home after establishing relationships with nursing homes and patients.

Responsibilities include triaging requests, conducting weekly visits, coordinating care, performing medical assessments, and managing health plans for residents.

Qualifications

• Registered with AHPRA as Nurse Practitioner.
• Endorsed Nurse Practitioner and eligible to practise in Australia.
• Able to apply for Medicare provider number.
• Maintain own professional indemnity insurance.
• Excellent collaboration with medical officers and care teams.
• Proficient with basic computer skills and patient communication.

Responsibilities

• Provide primary care within RACFs in NSW, supporting GPs.
• Triage requests and conduct weekly onsite visits.
• Coordinate care between GPs, aged care staff, residents and families.
• Perform health assessments and develop management plans.
• Conduct health reviews for new admissions and ongoing residents.
• Opportunity to work from home once established with facilities.
