Job summary
Access Aged Care is seeking a dedicated General Practitioner to deliver on-site medical care to residents in the Gosford and Wyong regions. Workweek is weekday hours only with dedicated nursing support to coordinate rounds, follow-up and care plans.
You will operate with telehealth geriatrics and specialists for complex decisions, aiming for proactive, relationship-based care and better resident outcomes. A laptop and tech proficiency are required.
Qualifications
- Registered as a General Practitioner with AHPRA.
- Fellowship with RACGP or ACRRM, or nearing completion.
- Minimum one day per week commitment; potential to add more sessions.
- Comfortable with technology and clinical software used in care coordination.
- Own laptop required to perform duties.
Responsibilities
- Provide on-site medical care to residents across Gosford and Wyong regions.
- Collaborate with an Access Aged Care Nurse for rounds and follow-up care.
- Coordinate care plans, investigations and home communications to streamline sessions.
- Work with telehealth geriatricians and specialists for complex decisions.
