Qualifications

• Registered with AHPRA as Nurse Practitioner.
• Endorsed Nurse Practitioner and eligible to practise in Australia.

• Excellent collaboration with medical officers and care teams.
• Proficient with basic computer skills and patient communication.

Responsibilities

• Provide primary care within RACFs in NSW, supporting GPs.

Nurse Practitioner (Batemans Bay to Dalmeny)

We are currently seeking an experienced Nurse Practitioner to join our clients primary care team and support our General Practitioners in providing care to patients residing in Residential Aged Care Facilities (RACFs) across:

• Batemans Bay to Dalmeny

Duties and Responsibilities:

• Work collaboratively with our Senior Medical Officer and General Practitioners and ensure the highest level of care is provided to our patients and reduce hospital admissions.
• Triaging incoming requests from the RACFs allocated to you and conducting weekly onsite visits.
• Coordinate care between GPs, aged care staff, residents, and families ensuring continuity of quality care.
• Provide primary care services which may include:
  • taking medical history
  • undertaking clinical examination
  • arranging any necessary investigations
  • implementing a management plan
  • providing appropriate preventive health care
• Contribute to health assessments and medication management for new resident admissions.
• Prepare annual health assessment reviews for existing residents.
• Conduct regular reviews and follow ups with aged care residents through professional attendance.
• There may be opportunity to work from home once relationships with the nursing homes and patients are established.

Skills and Abilities:

• Registration with AHPRA and endorsed as a Nurse Practitioner.
• Permitted to work in Australia.
• Hold or have the abilityto apply for a Medicare provider number.
• Hold your own professional indemnity insurance
• Great attitude and able to work in collaboration with our Senior Medical officer.
• Basic computer knowledge and good social / communication skills.
