Job summary

Access Aged Care is seeking a Nurse Practitioner to deliver full-scope advanced practice in Residential Aged Care Homes across Traralgon and Paynesville.

You will assess, diagnose, prescribe and care-plan for residents within a collaborative multidisciplinary model, supported by on-site nurses, GPs, telehealth geriatricians and physicians, with dedicated admin and operational support to keep focus on clinical care.

Qualifications

• Experience in aged care, gerontology, palliative care or chronic disease management.
• Advanced clinical skills in assessment, diagnosis, treatment and health promotion.
• Strong clinical reasoning, communication and leadership abilities.
• Knowledge of aged care legislation and regulatory requirements.
• Collaborative, resident-centred approach with tech proficiency.

Responsibilities

• Provide advanced assessment, diagnosis, prescribing and care planning for residents.
• Collaborate with on-site nurses, GPs, telehealth geriatricians and physicians.
• Deliver high-quality care with dedicated administrative support.
