Access Telehealth is seeking a Nurse Practitioner to deliver advanced aged care clinical services across Launceston and surrounding areas. You will conduct comprehensive assessments, make diagnoses, prescribe within scope, and develop care plans for residents in collaboration with on-site nurses, GPs, telehealth geriatricians and physicians.

This role offers flexible weekday scheduling for one regular day per week, with dedicated operational support to enable focus on high-quality clinical care

Qualifications

• Experience in aged care, gerontology, palliative care or chronic disease management is highly regarded.
• Advanced clinical experience in assessment, diagnosis, treatment, prescribing and health promotion.
• Strong clinical reasoning, communication and leadership capabilities.
• Experience in quality improvement and evidence‑based practice.
• Sound knowledge of aged care legislation and regulatory requirements.
• Current driver's licence, reliable vehicle and appropriate professional indemnity insurance.
• Comfortable using technology and clinical software systems.

The Role

Working across Residential Aged Care Homes in Launceston and surrounding areas, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.

What You'll Bring
