Job summary

Uniting Home Care in Western Sydney seeks a Registered Nurse to provide high-quality, person-centred nursing care in clients’ homes across Western Sydney. You will assess needs, develop and review care plans, deliver clinical interventions and monitor health changes to support safe, effective care at home.

You will work closely with clients, families and the care team, offering clinical guidance and coordinating care that promotes independence, dignity and quality of life.

Qualifications

• Current AHPRA registration and a nursing qualification.
• Experience in aged care or community nursing with interest in aged care.
• Commitment to contemporary, person-centred clinical practice.

Responsibilities

• Assess clients’ needs and develop/review care plans.
• Deliver clinical interventions and monitor health changes.
• Coordinate care with families, carers and the care team to promote independence and dignity.
