Outcomes Connect Australia (OCA) is seeking a Behaviour Support Practitioner to help people with disability improve quality of life through evidence-based practice and person-centred planning.

You will work with a team of practitioners, a team leader and clinical advisors, delivering plans that reflect clients' goals while supporting families and carers. Remote/community-based work and Warrnambool proximity are supported.

Competenze

• Registration with the NDIS Commission is preferred but not essential.
• Considerable practical experience supporting people with complex behaviours.
• Ability to work positively as part of a diverse team.
• Outstanding communication, influencing and interpersonal skills.
• Confident computer skills and clear report writing abilities.

Mansioni

• Identify needs through assessment and person-centred planning for NDIS participants.
• Develop and evaluate evidence-based, person-centred plans in a complex space.
• Align individual plans with client goals, passions and strengths.
• Build knowledge and capacity of families, carers and supporters.
