Job summary

Outcomes Connect Australia seeks an experienced Behaviour Support Practitioner to improve lives of people with disability through person-centred planning and evidence-based practices. You’ll work with a team of practitioners, a team leader and clinical advisors to support clients in Adelaide and beyond, applying PBS principles in home and community settings.

You live in Adelaide and will engage in ongoing professional development with our PBS program in partnership with leading academic

Qualifications

• Current suitability (i.e. registration) with the NDIS Commission as a Behaviour Support Practitioner is essential.
• Demonstrated ability to work positively in a diverse team.
• Outstanding communication, influencing and interpersonal skills.

Responsibilities

• Support our clients (NDIS participants), families and carers, to identify needs via assessment and person-centred planning.
• Identify meaningful goals and practical strategies using the Positive Behaviour Support framework.
• Develop, deliver and evaluate evidence-based support plans in a complex space.
• Ensure plans reflect clients' goals, passions and strengths.
• Build capacity of those who support clients — family, carers, friends, etc.
