CareChoice Group Ireland in Geelong, Australia, is seeking a dedicated Support Worker to provide person-centred support across SIL homes. You will assist with daily living activities, health supports, and community engagement, helping participants live as independently as possible.

The role requires Certificate III in Individual Support, 2 years with NDIS, and checks. You will work rosters, maintain records, and uphold dignity and choice while building trusting relationships.

Formación

• Minimum Certificate III in Individual Support (Ageing, Disability or related discipline).
• Minimum 2 years’ experience supporting NDIS participants with complex support needs.
• Experience supporting clients with Behaviours of Concern.
• Current NDIS Worker Screening Check and Working with Children Check.
• Current First Aid and CPR certification.
• Valid Victorian Driver’s Licence and access to a reliably insured vehicle.
• Availability to work the ongoing roster outlined above.

• Provide tailored, person-centred support promoting independence, wellbeing and quality of life.
• Assist with personal care, health supports and day-to-day living activities.
• Perform manual handling and transfers using prescribed equipment safely.
• Facilitate community engagement, appointments and social activities.
• Build a positive relationship while upholding choice and dignity.
• Maintain accurate records and communicate changes in needs or wellbeing.
