Job summary

Healthscope Independence Services in Geelong seeks a dedicated Disability Support Worker to provide high-quality personal care and daily living support to clients in SIL settings. You will assist with routines, meal prep, and community activities, ensuring person-centred care and safety.

The role requires a Certificate IV in Disability or related field and at least 1 year of relevant experience, with strong communication and organisational skills.

Qualifications

• Certificate IV in Disability or related qualification.
• Minimum of 1 year of experience in disability or healthcare services.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.

Responsibilities

• Provide personal care and daily living support per individual plans.
• Overnight sleepover shifts with on-site presence and morning handover.
• Assist with meal preparation and household tasks.
• Support participants with community activities and appointments.
• Document daily notes, progress reports, and communications.
• Adhere to policies including medication administration, WHS, privacy and confidentiality.
• Work collaboratively within a team environment.
