Job summary

CareChoice Geelong is seeking a dedicated Support Worker to assist people with disability across 24/7 Supported Independent Living homes. You will provide person-centred support enabling participants to live independently and achieve individual goals.

Successful applicants will have a Certificate III in Individual Support, 2+ years of relevant experience, and current First Aid/CPR, NDIS Worker Screening, and Working with Children checks. A Victorian driver’s licence is essential.

Qualifications

• Certificate III in Individual Support (Ageing, Disability or related discipline) required.
• Minimum 2 years' experience supporting NDIS participants with complex needs.
• Experience supporting clients with Behaviours of Concern.
• Current NDIS Worker Screening Check and Working with Children Check.
• Current First Aid and CPR certification.
• Valid Victorian Driver's Licence and access to a reliable vehicle.
• Availability to work the ongoing roster.

• Assist with personal care, health supports, and daily living activities.
• Perform safe manual handling and transfers using prescribed equipment and plans.
• Facilitate community engagement, appointments and social activities.

About The Role:

Join our Geelong Support Worker team and make a meaningful difference in the lives of people with disability. Based across our 24/7 Supported Independent Living (SIL) homes, you'll provide person-centred support that empowers participants to live as independently as possible and achieve their individual goals.

Your Duties and Responsibilities:

• Provide tailored, person-centred support that promotes the participant's independence, wellbeing, and quality of life.
• Assist with personal care, health supports, and day-to-day living activities in a respectful and professional manner.
• Perform manual handling and transfer duties safely, utilising prescribed equipment and following established support plans.
• Facilitate community engagement, appointments, and meaningful social and recreational activities.
• Build a positive and trusted relationship with the participant while upholding their choice, control, and dignity.
• Maintain accurate records and communicate effectively regarding any changes in the participant's needs, health, or wellbeing.

About You:
