Job summary

MMT Care Pty Ltd is seeking an experienced Support Worker to empower participants with complex needs. You will assist with personal care, including AFO brace management, and help with transitions between appointments. Training in Proloquo and sign language will be provided to support effective communication.

Applicants should bring calm, patient, and respectful approaches, with a commitment to documenting progress and working with families and support teams to achieve personal goals.

Qualifications

• Experience supporting participants with complex behaviours and Positive Behaviour Support strategies.
• Calm, respectful, person-centred approach to challenging or changing situations.
• Willingness to learn Proloquo and sign language (Auslan a bonus).
• Observe and record changes in wellbeing against the support plan and communicate clearly.

Responsibilities

• Provide person-centred support promoting dignity, choice, safety and independence.
• Assist with personal care, including helping the participant with putting on their AFO brace.
• Support transitions to and from appointments.
• Support participants with complex behaviours using calm, respectful strategies per the PBS plan.
• Use Proloquo and sign language to enable participant choice.
• Encourage participant to build confidence and stay connected with community.
• Complete accurate records and progress notes to support outcomes.
