Job summary

One Doorway is seeking a dedicated Disability Support Worker in Victoria to provide person-centred care in clients' homes, supporting daily living tasks and complex needs. You will work within a dedicated team to ensure safety, dignity and meaningful outcomes for participants.

You will collaborate with families and a multidisciplinary team, delivering advocacy, empowerment and high-quality support, including every day living activities and health-related tasks required by participants.

Qualifications

• Experience supporting participants with high and complex needs.
• Strong communication skills and keen attention to detail.
• Commitment to rights and meaningful, dignified lives for people with disabilities.

Responsibilities

• Deliver person-centred care and daily living supports based on individual plans.
• Provide complex care including tracheostomy care and PEG feeding as needed.
• Advocate for participants and empower them through choice and independence.
• Develop positive relationships with clients, families and stakeholders.
• Support behavioural and mental health needs as part of care.
