Job summary

Healthscope Independence Services (HIS) is seeking a dedicated Disability Support Worker in Wyndham Vale, VIC. You will support clients to live independently, assist with daily living activities, personal care, and community engagement, following individual support plans.

Ideal candidates have 1+ year experience, a Certificate III/IV in Disability or related field, strong communication, and flexibility for shifts.

Qualifications

• Certificate III/IV in Disability, Community Services, or related qualification.
• Minimum of 1 year of experience in disability or healthcare settings.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.
• NDIS screening check, First Aid, CPR, Driver’s Licence, and evidence of right to work in Australia.

Responsibilities

• Provide personal care support and assistance with daily living activities.
• Support participants with meal prep, feeding, and daily tasks.
• Assist with household tasks such as cleaning and laundry.
• Accompany clients to community-based activities and appointments.
• Maintain thorough documentation including daily notes and progress reports.

Job description

At the Healthscope Independence Services (HIS), we are seeking a passionate and experienced residential/ Attendant Care Support Worker to provide high‑quality support to our clients receiving Attendant Care services under the NDIS.

Classification: Level 2 – (Pay point will be determined based on experience and qualifications).

Work Location: Wyndham Vale, VIC and nearby surrounding suburbs.

About the Role

As a Disability Support Worker, you will be responsible for providing high-quality care and assistance to our clients, helping them achieve their goals and live independently. You will work closely with our clients, supporting them with daily living activities, personal care, and community engagement.

A typical day may include:

Provide personal care support in line with individual support plans

Assist participants with daily living activities in meal preparation and feeding.

Assist with household tasks such as cleaning, laundry, etc

Accompanying and supporting clients in community-based activities, such as social outings, appointments, and recreational pursuits

Ensure all required documentation (including daily notes, communication and progress reports) is thorough and up to date.

Experience with behaviours of concern

Adhere to all organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality.

To be considered for this role you will have:

Certificate III / IV in Disability, Community Services, or related qualification.

Minimum of 1 year of experience in disability or healthcare services is required.

Strong communication, written, interpersonal, and organisational skills.

Flexibility to work across a range of shift types.
